How long is the Consent from valid for?
When requesting GCMS electronic notes under the Access to Information Act, you will need to sign a consent form (IMM 5744). The most recent format of the consent form is valid for 1 years from the date it was signed. If you have placed an order for GCMS notes with us, and provided us with a consent form, we can use it for the subsequent order. However, we will need your express consent to use your previous consent form. Also, you will have to send it again to us by email. To be in compliance with the The Personal Information Protection and Electronic Documents Act (PIPEDA), we do not retain copies of consent form once they have been transmitted to IRCC.
